Using mixers on the IO side had a remote benefit of being able to
override all control surfaces with a radio remote on a fixed wing.
This ended up not being used that much and since the original design
10 years ago (2011) we have been able to convince ourselves that the
overall system stability is at a level where this marginal benefit,
which is not present on multicopters, is not worth the hazzle.

The documentation of the thrust model parameter `THR_MDL_FAC` did not
mention both thrust and "PWM" being relative values. Also the use of the
term PWM could be misleading, since the model is applicable to CAN ESCs
as well.
This commit rephrases the user documentation string and a few source
code comments, but no logic changes are made.
ClosesPX4/Firmware#13105
since pure manual control for fixed wings in handled on the io side
the scale parameters for roll, pitch and yaw had to be introduced there
as well.

__STDC_FORMAT_MACROS changes the behavior of inttypes.h to allow
defining format macros for printf-like functions. It needs to be defined
before any include is done, otherwise due to include chains and header
guards it may not take effect.
Instead of having to define it everywhere it is used, move the define to the
build system. Also update ecl and DriverFramework submodules to deal with the
changed definitions.
this allows the flight code to choose whether FMU failure gives
immediate manual pilot control, or waits for the mode switch to go
past the override threshold
this allows the FMU to force the safety off on the IO board. Useful in
two cases:
1) vehicles where the safety switch is impractical or not useful
(eg. HAB planes or internal combustion motors)
2) doing ESC calibration on multi-copters
